    How to Fill Out Your Live Scan Form

    1. STEP 1

      Confirm the requesting agency (employer, licensing board, or government department) and the Originating Agency Identifier (ORI) — they must provide it.

    2. STEP 2

      Select the correct form. Most applicants use the BCIA 8016; record review and immigration requests use the RR or CV variants.

    3. STEP 3

      Complete the Applicant Information section: full legal name, date of birth, sex, height, weight, eye and hair color, place of birth, Social Security number, and driver's license number.

    4. STEP 4

      Enter the requesting agency information exactly as provided (agency name, address, contact name, ORI, authorized applicant type, and any billing or mail codes).

    5. STEP 5

      Print three copies — one for the applicant, one for the requesting agency, and one for the Live Scan operator.

    6. STEP 6

      Bring all three printed copies plus a valid government-issued photo ID to your American Health Biometrics appointment.
